1.1 When you contact Axis by phone call, online call or by making enquiries with our personnel, then by virtue of recording that interaction, we may collect Personal Information (including Sensitive Information) about you or another individual.

1.2 We may require you to speak to a number of Axis’s personnel, including treating Practitioners, and those enquiries will be recorded too. If you provide Personal Information about another person, you confirm that you have that person’s consent to share their information in this manner.

2.1 We may use your Personal Information in the ways and for the purposes contemplated in our Privacy Policy, which includes to enable us to operate our business, provide the Services to you, or as otherwise required or authorised by law.

2.2 We may also convert this call and future call recordings into a transcript to facilitate training and quality assurance processes. Where possible and practicable, when using Personal Information for training and quality assurance purposes, we will de-identify this information.

2.3 We may disclose your Personal Information to our personnel, treating practitioners, and other third parties involved in your rehabilitation including your treating doctor or specialist, allied health therapists, employer, or injury management stakeholders for the primary purposes of providing quality assessment, diagnosis, treatment, and facilitation of a successful rehabilitation outcome.

2.4 We will never disclose your Sensitive Information to your employer for the purpose of facilitating litigation, court proceedings or similar, unless we have your express consent.

2.5 We may also disclose your Personal Information to our employees, contractors, and third parties who assist us in providing our services and operating our business, or as otherwise required or authorised by law. 3. Accessing or correcting your information

3.1 You can always decline to give Axis any Personal Information we request, but that may mean we cannot provide you with some or all the services you have requested.
