Terms and Conditions

Please see below terms and conditions applicable to the service you are using:

Onsite Physiotherapy

  • Please understand that you have the choice to see a doctor at any time and choose which health providers you see.
  • Despite attending Axis, you must still comply with your employer’s injury reporting requirements.
  • If your injury/illness is work related you also have the right to lodge a workers compensation claim, through discussion with your employer.

Onsite Psychology

  • Please understand that you have the choice to see a doctor at any time and choose which health providers you see.
  • If your injury/illness is work related you also have the right to lodge a workers compensation claim, through discussion with your employer.

Private Practice Physiotherapy

  • I understand that this practice does NOT operate on terms of an Account basis and I agree to provide payment in full for service provided on the day of consultation.
  • Should you have physiotherapy treatment eligible under a WorkCover or Insurance claim, unless prior arrangement has been made with your physiotherapist, it is an Axis Physiotherapy Practice Policy to charge you (the client). It is then your responsibility to lodge the claim through WorkCover or your insurer.
  • Axis have a HICAPS terminal and can process private health insurance claims on the day, however it is your responsibility to confirm your eligibility and level of cover with your private health insurer.
  • If I have a Workcover or insurance claim which is not accepted, I agree to pay for all the treatments not covered, at the standard physiotherapy rates.

Private Practice Psychology

Axis Psychology Service Agreement – Commitment to Treatment:

  • Our commitment is to provide evidenced based treatments, to care for you with empathy, compassion, and respect. We are required by law to take notes either during or at the end of sessions. These notes are handled with care and stored securely. We will respect your privacy and uphold our commitment to confidentiality (with some exceptions which your psychologist will explain).
  • If coming under a GP MHCP we are also required to communicate your engagement in treatment with your referring GP. Please ask if you have any questions about the nature of the information shared.
  • We will dedicate 50 minutes per session to you. Appointments are booked on the hour, but your appointment includes the administration time and planning of your treatment.
  • The cost of your sessions will have been explained to you, if they haven’t or if you have any questions or concerns about payment, please discuss these with your treating psychologist.
  • If coming under a GP MHCP your psychologist will notify you when a review with your GP is required, if you would like regular updates on how many sessions you have please let your psychologist know.
  • Your commitment is to be open and honest and to engage in the process as best you can.
  • If you no longer require your appointment, you are required to give notice to ensure we can also provide treatment to others in need. We explain this in detail in our cancellation policy.
  • Unless prior arrangement has been made, you are required to pay in full for each session on the day.

Axis Private Practice Psychology Cancellation Policy

Axis reserves the right to charge a cancellation fee of 50% of the consultation cost to the individual for missed or cancelled appointments where adequate notice has not been provided (this will not be covered by third party funding organisations). To avoid incurring a cancellation fee, clients are asked to provide at least 24 hours notice if they wish to cancel or reschedule their appointment. Axis also reserves the right to deny rescheduling services until cancellation fees have been rectified.

Please note: No Medicare or health fund rebates are available for cancellations. We are not a Bulk Billing practice.
